LOCK HAVEN - Community Nursing Service of Clinton County and Honeywell HomMed, a global leader in telehealth and remote patient care, are collaborating to implement a comprehensive telehealth solution designed to streamline Community Nursing Service's employee workflows and improve overall patient care.
Telehealth monitoring will be an additional component of the Community Nursing Service's "Chronic Care Management Program."
The program is staffed by Advance Care Specialists, utilizing evidence-based guidelines in developing customized patient care plans that significantly decrease patient rehospitalizations .
Community Nursing Service has demonstrated its dedication to maintain high quality care for the community by establishing a new system wide Advance Care Specialist program to enhance the knowledge of the clinical staff.
