LOCK HAVEN - Lock Haven Hospital has announced the facility's Extended Care Unit has undergone a name change and will become Haven Skilled Rehabilitation and Nursing.
Haven Skilled Rehabilitation and Nursing will host a public open house from 4:30 to 6 p.m. on June 21. Barbara Nicklaus for Southern Care will do a presentation on advance care planning and Chris Benfield from Select Rehabilitation will do presentation on back safety. Light refreshments will be served and there will be a drawing for door prizes.
"The Extended Care Unit has served a variety of needs for patients in the Lock Haven community, ranging from traditional long term stays to post-operative recovery and hands-on therapy patients," said John Zidansek, chief executive officer, Lock Haven Hospital. "We feel that Haven Skilled Rehabilitation and Nursing gives clarity to the range of professional services that we have to offer."
Haven Skilled Rehabilitation and Nursing at is a 90-bed, long term care facility. HSRN is accredited by The Joint Commission and is licensed by the Pennsylvania Department of Health.
Through carefully planned rehabilitation, patients can recover or increase their ability to perform daily activities. HSRN can ease worries and provide the rehabilitation services to help get patients back on their feet and back to life. After completion of an acute-care stay, the HSRN program allows patients to rehabilitate close to home, from any acute care facility.

Rehabilitation and nursing offers a progressive restorative nursing program that is instrumental in supporting resident's ability to maintain their independence. All residents have an individualized plan of care developed with the resident's family involvement and coordinated by a registered nurse, primary care provider and therapy team members.
