Rotary Club offers blood testing in April
FLEMINGTON — The Rotary Club of Lock Haven is sponsoring its 50th annual health screening Saturday, April 5, from 6 to 10 a.m. in Flemington First Church of Christ at 714 Canal St.
The AMBA Wellness Program offers a full panel consisting of tests for coronary disease, diabetes, anemia, kidney disease and liver disease. Cholesterol, red blood cell count, white blood cell count and glucose are a few of the many tests in this panel. This panel costs $44, with a separate fee of $10 to the host club.
Individuals taking the tests are required to fast, not eating any food or beverages, for 13 hours prior to their scheduled appointment time.
Also available at additional cost are five specialized tests. AMBA requires that a separate physician order must be presented for any of the optional tests, which are: Prostate Specific Antigen (PSA) for $27, Thyroid TSH for $13, Vitamin D-25 for $26, Hemoglobin A1C for $13 and Insure Colon Rectal Kit for $22.
None of the screenings are covered by Medicare or most insurance companies.
Payment for the AMBA tests is to be made with a check or money order payable to AMBA Wellness Programs. Cash will not be accepted.
An additional separate fee of $10, payable in cash or by check, will be charged by Rotary to cover the costs of advertising and light refreshments for individuals having the blood screening. Rotary will use any proceeds remaining after expenses to make charitable contributions within the community.
You must schedule an appointment for this testing and may do so by calling 1-800-234-8888 between 8:30 a.m. and 5 p.m., Monday through Friday. Callers will be asked their physician’s name, address and phone number at the time of the call. Your doctor’s authorization is required, so call your doctor first before you call for an appointment.
If you would like a copy of your testing results, a PHI (Protected Health Information) form is needed, and you must show your photo ID.
